Acne

Elevated Red Blood Cell Count (Polycythemia)

Research on the occurrence of polycythemia as a side effect of testosterone therapy is limited, yet polycythemia is recognized as a potential risk of TRT.

Polycythemia is marked by an excessive increase in red blood cells. It may elevate the risk of vascular issues, such as stroke, high blood pressure, deep vein thrombosis, or pulmonary embolism. However, these complications, while theoretically possible, have not been conclusively linked to men undergoing TRT.

Due to the potential risk of polycythemia, we perform thorough and regular monitoring for men receiving TRT. We typically ask for a complete blood count (CBC) before initiating therapy to establish a baseline and monitor it regularly during treatment.

Prostate Health

A key risk factor associated with testosterone supplementation is its impact on the prostate. The prostate is an androgen-dependent gland, and anti-androgen agents can reduce prostate volume in patients with benign prostatic hyperplasia, a noncancerous condition that causes the prostate gland to grow larger than normal.

While testosterone therapy in older men may lead to an increase in overall prostate size, this increase is similar to the enlarged prostate observed in elderly men who are not receiving testosterone therapy. Overall, TRT is not contraindicated in men diagnosed with benign prostatic hyperplasia.

Historically, there were concerns that higher levels of testosterone may increase the risk of developing prostate cancer, causing worry about the safety of TRT. However, research now shows that additional testosterone does not increase the risk of prostate cancer.

The administration of TRT should be taken with caution for previously treated for high-risk prostate cancer, as the prostate is an androgen-dependent gland. It means that the prostate requires testosterone to grow, develop, and function normally.

Cardiovascular Concerns

Research into the cardiovascular effects of TRT has produced a range of findings, with some studies suggesting potential benefits while others highlight possible risks.

On the positive side, TRT, when combined with regular physical exercise, may offer cardiovascular benefits. Evidence suggests that TRT can help reduce body fat and increase lean muscle mass, which in turn can improve overall heart health by promoting a healthier body composition.

However, there are also concerns associated with TRT, particularly in terms of its potential to increase certain cardiovascular risks. Studies indicate that TRT can elevate red blood cell levels, which may increase the risk of blood clots. Elevated red blood cell counts can lead to thicker blood, raising the likelihood of clot formation, which could, in turn, result in complications like deep vein thrombosis, pulmonary embolism, or stroke.

Additionally, TRT may contribute to higher blood pressure, which places additional stress on the cardiovascular system. The effect of increased blood pressure can exacerbate the risk of heart disease and related conditions, especially in people with pre-existing hypertension or other cardiovascular issues.

Kidney & Liver Concerns

Sleep Apnea

Obstructive sleep apnea (OSA) is a potential risk associated with TRT in men. However, its exact cause remains unclear. While some studies have found no connection between OSA and TRT, others suggest that OSA develops in men receiving TRT and resolves once supplementation is discontinued.
